1. Indications and Usage
Severe Infections: Treatment of moderate to severe respiratory tract infections, complicated urinary tract infections, pyelonephritis, acute bacterial prostatitis, complicated skin and skin structure infections, intra-abdominal infections, and septicemia.
2. Dosage and Administration
Adults: The usual dosage is 400 mg once daily or 500 mg to 750 mg once daily. For severe infections or less susceptible pathogens, up to 600 mg to 750 mg once daily may be administered.
Renal Impairment: Dosage must be adjusted based on creatinine clearance to prevent drug accumulation.
Administration: Administer via intravenous infusion diluted in 5% glucose or 0.9% sodium chloride. The infusion rate should be controlled (typically not less than 60 minutes for 100 mL). Rapid infusion may cause CNS reactions.
3. Mechanism of Action
DNA Gyrase Inhibition: Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, thereby inhibiting bacterial DNA replication, transcription, repair, and recombination.
Spectrum: It demonstrates broad-spectrum activity against many aerobic Gram-positive and Gram-negative bacteria.
4. Safety and Warnings
Tendon Disorders: Fluoroquinolones increase the risk of tendinitis and tendon rupture in all age groups. Risk is further increased in elderly patients and those taking corticosteroids.
CNS Effects: May cause CNS stimulation, including tremors, anxiety, and seizures, especially in patients with a history of seizures or CNS disorders.
QT Prolongation: May prolong the QT interval; caution is advised in patients with cardiovascular risk factors.
Photosensitivity: May cause severe photosensitivity reactions; patients should avoid excessive sunlight or UV exposure.
5. Adverse Reactions and Clinical Research
Common: Nausea, vomiting, diarrhea, abdominal pain, headache, dizziness, and injection site reactions.
Serious: Tendon rupture, seizures, peripheral neuropathy, QT prolongation, and severe photosensitivity.
Clinical Research: Clinical trials have demonstrated high efficacy and safety in treating various community-acquired and nosocomial infections.
6. Drug Interactions
Antacids/Multivitamins: May significantly reduce the absorption if used concomitantly.
NSAIDs: May increase the risk of CNS stimulation and seizures.
Warfarin: May enhance the anticoagulant effect; prothrombin time should be monitored.
Theophylline: May increase serum theophylline concentrations.
