1. Indications and Usage
Licancopan fumarate capsules are indicated for the treatment of adult patients with paroxysmal nocturnal hemoglobinuria (PNH) who have not previously received complement inhibitor therapy.
2. Dosage and Administration
Recommended Dose: The standard recommended starting dose is 400 mg taken orally once daily.
Administration: The capsules should be swallowed whole with water. They can be taken with or without food, but should be taken consistently in relation to meals.
Missed Dose: If a dose is missed, it should be taken as soon as remembered unless it is close to the time of the next dose. Do not double the dose.
Dose Modification: Dose interruptions or reductions may be required for the management of severe adverse reactions, such as serious infections or liver toxicity.
3. Mechanism of Action
Licancopan is a selective, oral inhibitor of the complement component 5 (C5). By binding to C5, it prevents its cleavage into C5a and C5b, thereby blocking the formation of the membrane attack complex (C5b-9). This inhibits complement-mediated intravascular hemolysis, which is the central pathophysiological mechanism of PNH.
4. Safety and Warnings
Infection Risk: Inhibition of C5 increases the risk of serious infections with encapsulated bacteria (e.g., Neisseria meningitidis). Patients must be appropriately vaccinated against meningococcus prior to initiation and monitored closely for signs of infection.
Thrombosis: PNH carries a high intrinsic risk of venous thrombosis. Patients should be monitored for thrombotic events, and anticoagulation therapy should be considered based on clinical judgment.
Hematologic and Hepatic Monitoring: Regular monitoring of complete blood counts, reticulocyte counts, lactate dehydrogenase (LDH), and liver function tests is required to assess hemolysis status and drug safety.
5. Adverse Reactions and Clinical Research
Adverse Reactions: The most common adverse reactions (≥10%) observed in clinical trials included nasopharyngitis, headache, nausea, back pain, and anemia.
Clinical Research: In pivotal clinical studies, licancopan demonstrated significant and sustained reduction in lactate dehydrogenase (LDH) levels, transfusion requirements, and thrombotic events in patients with PNH, showing comparable efficacy to existing complement inhibitors.
6. Drug Interactions
CYP450 Interactions: Clinical studies have shown that licancopan is not a significant inducer or inhibitor of major CYP450 enzymes. However, caution is advised when co-administering with strong CYP3A4 substrates.
Concomitant Medications: No specific dose adjustments are generally required for most concomitant medications, but the effects of combining with other immunomodulatory or anticoagulant therapies should be monitored.
